@n3am I am not a medical specialist, but I know dandruff is quite common (I had a nasty case in middle school). But I can say that not all doctors are created equal, and if you're not getting the results you want, go to another. If you're locked into a specific person through and HMO or the like, write a polite but pointed letter their medical director.

For many years I had a very persistent dandruff and nothing seemed to be able to help - anti-dandruff shampoos were making it even more intense. The only shampoo that actually helped me (though not definitely I still get it from times to times) is the Selsun Blue shampoo. It's a medical shampoo not a cosmetic product - don't expect a pleasant smell (smells like a drug) but so far this is the only product I found that actually releaved my situation.
In the US it's available for sale but they stopped production and import in the EU. There is a similar product containing the same active substance - selenium teluride available for sale in Europe which is called Selmax Blue but it's less effective though still does the job. If you decide to try it I recon you somehow get the US product otherwise try the Selmax Blue it's available in Amazon for example.

@jujulette
yes i tried Selsun Blue once and i was best result i ever get
but in my country it's really expensive -
I am buying Selmax Blue since 2024. I am located in Luxembourg, Europe. Selsun is too expensive because it is commercialised only in the USA now.

Your severe white flakes are mostly caused by fungi overgrowth and seborrheic dermatitis, which is why it only improves with daily use.
You don’t have to worry too much about daily use destroying hair follicles.
Normal anti-dandruff shampoos (like those containing zinc pyrithione, selenium sulfide, or ketoconazole) are safe for daily short-term use. They clean the scalp surface and don’t go deep enough to damage follicles.
Use it daily until it gets better, then slowly reduce frequency.
Use the shampoo every day for 1–2 weeks until the white flakes almost disappear. Then switch to 2–3 times a week to maintain it. That’s how most people control it.
Don’t apply vinegar too strongly or too often.
Apple cider vinegar can irritate the scalp if used daily. It’s better to use it only occasionally, if at all.
Check your beard too.
The skin under your beard can also have seborrheic dermatitis. Wash the beard gently with the same anti-dandruff shampoo, and avoid scratching.
If it still doesn’t improve, see a dermatologist again.
You may need a mild topical steroid or special prescription lotion for a short time. -
The majority of antidandruff shampoos are made to treat the fungal variety, which is caused by oily/damp scalp.
But there's a type of dandruff that is caused by your scalp being so dry, that it sheds skin flakes.
I used to have this type, without knowing it. Used a bunch of differend shampoos and treatments, even a very expensive one recommended by a dermatologist, all without result. What fixed my problem for good, was actually very simple: Head & Shoulders - Hydration
Here in Brazil, is the one with Almond Oil, but I think there's also one with Coconut Oil.
I use it everyday and never had dandruff again.
If you think your dandruff is due to dryness on the scalp, it might be worth a try.
