First of all, if it hasn't been already communicated, a dude should let you know he's gonna blow so you can pull off if you're not into swallowing. Swallowing or spitting doesn't affect STD transmission at all. You're just as likely to contract any STD from spitting as you are from swallowing.
It may also be worth mentioning that HIV in particular is very unlikely to be acquired through oral sex. I've heard professionals say that it would require openly bleeding sores in the mouth of the bj giver to acquire HIV from a bj, in which case, a blowjob might not be enticing to the receiver in the first place.
To answer the question, I think it's confusing, rather than rude, why anyone would spit. Especially gay men. Cum is a part of sex, and it's difficult for me to imagine why someone would want to spit cum, if they're already comfortable with sucking dick.
