I would love to know the back story here, which is likely edited out from the final. It's not unusual for a condom to break, but that's at the tip. It would seem that in the bareback scene the condom would have to have been consciously removed (or the guy went soft and it slipped off), which would have been noticed by the director and/or camera people.
